Ua-arthorn policy

What is Ua-arthorn accidental insurance?

       This policy has been specially made due to a government initiative to help low income people from accident death or permanent of family members. There are several life insurance companies and non-life insurance companies working as the insurers which are managed by Thai Reinsurance PCL under affordable premium payment.


The benefit of Ua-arthorn Policy

       1. In case of death or permanent disability

    within 365 days from the day of the accident. The beneficiary or the insureds will be paid 300,000 baht. If both the insured and their spouse have died or received permanently disability, the beneficiary or the insureds will receive an addition of 100,000 baht. For a total of 400,000 baht.

                  Disability means the insureds are unable to work permanently or they have lost both hands, both feet or both eyes.
                  Hands and feet losses means Both hands or feet have been severed. The insureds are unable to use them to work a permanently profession which has been approved by doctor.
                  Blindness The insureds have been permanently blinded which is uncurable.

        2. In case of Death by illness

The company will pay the amount of 10,000 baht for funeral
        
Notice :Policy will pay only one benefit to the insured or beneficiary.

Premium payment
       There are 2 rates of premium:
  1.Ua-arthorn accidental insurance with a 365 baht premium payment.
2.Ua-arthorn accidental insurance plus funeral coverage with a 500 baht premium payment.

Required Documents
      
Copy of I.D. card with his/her signature and the name of only one beneficiary.

Qualification of eligible insured
      
Between 15-70 years of age with a copy of his/her I.D.card.

Conditions
      
1.The insureds are able to purchases either Ua-arthorn accidental insurance policy or accidental insurance plus funeral coverage. The insureds can not purchase funeral coverage only.
      2.In case the insureds purchase Ua-arthorn accidental insurance and they would like to buy funeral coverage in addition : They can do this at the time of extending their policy the following year.
      3. The insureds can purchase only 1 policy.
      4.The insureds and insurers can not cancel the policy.
      5.There is a waiting period for buying Ua-arthorn accidental insurance premium plus funeral coverage :
             5.1 There is a 6-month waiting period to buy Ua-arthorn accidental insurance plus funeral coverage in case of death from illness within the first 6 months of the policy. The insureds will not be paid for a funeral coverage. The insurer will refund only 135 baht premium to the beneficiary.
             5.2 When renewing policy without a waiting period:
                     - For Ua-arthorn accidental insurance plus funeral coverage: The insureds can renew their policy for next year but within 30 days from the date that the previous policy has expired.
                     - For Ua-arthorn accidental insurance: The insureds can renew their policy for the next year with additional funeral coverage but within 30 days from the date that the previous policy has expired
                     
      6.Eligible payee
              In case of death : The beneficiary or direct relative will recieve the coverage amount.
              In case of permanently disability : The insureds will recieve the coverage amount.

Claim examptions for Ua-arthorn accidental insurance
      
Ua-arthorn accidental insurance will not cover death or permanent disability from;
      
1. If insureds are intoxicated under the influence alcohol , or drugs
           "Under the influence the alcohol means the level of alcohol in the blood is more than 150 milligrams/millilitre"
      2. Suicide, Attempted suicide or self-mutilation.
      3. Contracted diseases except tetanus or rabies which are were resulted by accident.
      4. Miscarriage except miscarriaging by accident.
      5. War (Be declared or not), invasion, genocide, revolution, treason, riot against government, strike and terrorism.
      6. Nuclear weapons, nuclear radiation, nuclear fusion.
      7. The insureds is in the act of commit a crime, is being under arrested or detained by police, is a fugitive or has been sentened to 3 years or more imprisonment.
      8. The insureds are military personnels, police officers or volunteers that have gone to war.


The place to purchase Ua-arthorn accidental insurance
       *For people who live in the countryside they can contact their village leader or village headman.
       *For people who live in Bangkok or municipality. They can contact the joined insurance companies. There are 50 companies which comprise of 41 non-life insurance companies and 9 life insurance companies as follows
              1. Thai Life Insurance Company
              2. American International Assurance
              3. Ayudhya Allianz C.P. PCL
              4. Bangkok Life Insurance
              5. Muang Thai Life insurance
              6. Nationwide Life Insurance
              7. Siam Commercial New York Life PCL
              8. Millea Life Insurance (Thailand) company
             

Certificate of Insurance
       * White Certificate : for Ua-arthorn accidental insurance



      * Blue Certificate : for Ua-arthorn accidental insurance plus funeral coverage .

Claim submitting for Indemnification
       When accident has occurred, the insured or beneficiary must inform the company for indemnification by calling the nationwide hotline 1356.

Documentation for indemnification

       1. In case of death : All documents must be sent within 30 days after the date of death.
           - Death certificate
           - Autopsy certificate or copy of death certification from hospital
           - Copy of police report.
           - Copy of marriage certification (in case of both insured and spouse lost their lives to the same circumstances)
           - Copy of I.D. card of beneficiary or direct relative

       2. In case of permanent disability : All documentation must be sent within 365 days after the date of accident.
           - Original copy of doctor's report.
           - Copy of police report.(if any)
           - Photographic evidence to show the insureds permanent disability at the day of the claim.
           - Copy of marriage certificate (In case of both spouses have lost their lives)
           - Copy of insured's I.D. card

       Autopsy or Doctor Examination

When considering to pay indemnity, the company has right to ask for an autopsy and will pay for all expenses incurred this procedure.

       Payment of claim indemnification
The company will pay directly to the insureds, beneficiary or direct relative through the bank. They will receive it within 20 days after the company has received complete information. If the company is unable to pay on time, the company must pay an additional 7.5 % interest per year of the amount of payment from due date.
